Sponsorship forms to download

I'm currently in the process of making some downloadable forms that people can download, print off & then take to work, schools or their bike clubs to get people to sponsor them for doing the runs.

The idea is then once your mates have sponsored you to take part in the run, you can collect the money & a tear-off section will be added to include with the money you have raised to hand in on the day or to send in the post.

If anyone has any thoughts or ideas on this, please let us know.
